When patients are discharged from the hospital, they often need an intermediate level of care before returning home. Baptist has a Transitional Care Center to meet that need. The 18-bed facility provides a transitional level of care for patients who need assistance before going home, to a rehabilitation center or nursing home.

Staff use a comprehensive approach to caring for each patient, and an activities director creates a plan of activities that is both entertaining and care-related. Physician referral is required, and transitional care is reimbursed by Medicare.
