Geriatric Assessment Program

Seniors and their families have a place to turn when in need of mental health services. GAP is an outpatient program that involves physicians, who specialize in geriatric mental health, nurse practitioners, registered nurses and social workers. GAP evaluates patients who are age 55 and older who suffer with dementia, depression, anxiety and other mental disorders.

The program is designed to detect early signs and symptoms of memory problems and provide early intervention. The program's treatment approach emphasizes wellness, controlling risk factors, social interaction, maintaining strength and long and short term planning. By educating, supporting and counseling both the family and the patient, GAP is able to help everyone involved.
